Swimming and Diving State Meet Forms

Swimming and Diving Scratch Form UIL will handle all call-ups due to scratches. 

Girls Swimming Relay Team Confirmation Form **Each school must submit this form for their school PRIOR to the State Meet. Coaches will have the opportunity to verify/change relay members upon arrival to the swim center. SUBMIT BY 2/7 (4A and Below) and 2/14 (5A & 6A)

Dive Film Pass Schools who have a diver with the NISCA All-American Qualification Degree of Difficulty (Girls – 13.0 DD total for Optionals; Boys – 13.2 DD total for Optionals) are allowed to appoint ONE individual per qualified diver to videotape for the purpose of All-American Diving qualifications submission. This form MUST be submitted PRIOR to February 11th (4A) and February 19th (5A/6A).
